I distribute this survey to gather information about students' feelings regarding the summer research experience. I use this information to improve the summmer experience for future students and as a source of information for my report to the dean. I ask you to write your name on the survey because I find the information makes more sense when I see it in the context of a particular student. I may also meet with you sometime to discuss your answers.
You should be honest on this form and in those conversations. Neither negative nor positive comments will affect your grade in the class or our future interactions.
